1. Download Omne by FWD from October 1 to 31, 2024.

2. Create an account using either your mobile number or email address.

3. Link your policy under “FWD Insurance” tab and get a chance to win one (1) brand-new Samsung Galaxy Z Flip 5.

Drawing of the winner: 

• Drawing of the winner will be on November 29, 2024.
• One (1) lucky winner will receive a brand-new Samsung Galaxy Z Flip 5.

Distribution of the Prize: 

• The winner will be contacted via email, SMS, and/or phone call by FWD Life Insurance Corporation (“FWD”).
• The winner will have an option to either:
  • Pick-up their prize at FWD main office located at 19/F W Fifth Avenue Bldg., 5th Avenue cor. 32 Street, Bonifacio Global City, Fort Bonifacio, 1634, Taguig, Metro Manila; or
  • Have it delivered to their preferred address through LBC or Air 21.
• FWD will arrange the pick-up and/or the delivery of the prize upon confirmation of the winner’s preferred schedule.

FAQs 

• Is this available nationwide? - All FWD customers, excluding FWD employees, with an active policy are eligible to join.
• Are multiple people who live in the same address eligible to join? Yes, anyone with active FWD policy physically located within the Philippines can join.
• What is the campaign period? This campaign will run on October 1 – 31, 2024.
• How can the winner claim their prize? FWD will contact the winner and confirm his/her preferred delivery schedule or pick-up date. FWD can arrange delivery or the winner can pick up their prize at FWD main office.
• How will the winner be notified? The winner will be contacted via email, SMS, and/or phone call by FWD.
• How many raffle entries can I earn? Policy linking requires the use of the Customer ID which is unique to every user. Each customer can only earn one (1) raffle entry during the promo period. Multiple policies will only be considered as one (1) entry.

   1. This Campaign is launched and handled by FWD Life Insurance Corporation (“FWD”).
   2. The Campaign is open to all active FWD policyowners physically located in the Philippines. Each customer can only earn one (1) raffle entry during the promo period. Multiple policies will only be considered as one (1) entry.
   3. Campaign duration: October1 – 31, 2024.
   4. No purchase is required to join this Campaign.
   5. FWD reserves the right to cancel or amend the Campaign and these terms and conditions. Any changes to the Promo will be shown in the terms and conditions published on FWD's official website (fwd.com.ph).
   6. Validation of the winner will be held at the FWD main office located at 19/F W Fifth Avenue Bldg., 5th Avenue cor. 32 Street, Bonifacio Global City, Fort Bonifacio, 1634, Taguig, Metro Manila. The winner will be drawn by raffle.
   7. FWD’s decision, to be done by or in the presence of three (3) FWD representatives, is final and irrevocable.

4. Upon entering the Campaign, the Participant:

1. 1. Warrants that he/she has read and understood and that he/she agrees to be bound by these Terms and Conditions;
   2. Consents and allows to the processing by FWD of his/her personal data in accordance with FWD’s privacy policy at https://www.fwd.com.ph/privacypolicy/, for the purpose described above;
   3. Agrees to hold FWD free and harmless from any loss, injury, or damage caused by or arising from his/her participation in the Campaign or caused by or arising from his/her acceptance and/or use of the prize;
   4. Agrees to cooperate and participate in any advertising or publicity activities of FWD relating to the marketing coverage; and

If chosen as the winner, the Participant agrees that FWD may publicize the same in such mode and manner as it considers fit.
