For easier process, kindly prepare the following documents:

Completely accomplished, dated and signed Claimant’s Statement forms of the beneficiaries
Download Claimant statement forms
Original copy of the Certified True Copy of the Death Certificate of Insured issued by the Local Civil Registrar (LCR) or Original copy issued by the Philippines Statistics Authority (PSA)
Copy of Claimant’s government ID with date of birth, photo and signature

Submitting optional documents can expedite the processing of your claims.

Proof of relationship (e.g. Marriage certificate or birth certificate by the Philippines Statistics Authority (PSA)
Proof of relationship (e.g. Marriage certificate or birth certificate by the Philippines Statistics Authority (PSA)
Insured’s government ID with date of birth, photo and signature
Attending physician’s statement
Proof of bank account with full name and account number visible (e.g. Statement of account, ATM card, validated deposit slip, etc.)

Additional documents if death is due to violence/accident:

Completely accomplished, dated and signed Attending Physician’s form
Download Attending Physician’s form
Police report or police traffic incident report
Driver’s license if accident happened while the insured is driving
Complete medical/hospital records of insured from previous and recent consultations and confinements (Including but not limited to Medical Certificate, Admitting History Record, Clinical Abstract, Discharge Summary, lab result and all diagnostic results.)
